A passport is an internationally recognized travel document that verifies the identity and nationality of the bearer. A valid United States passport is required to enter and leave most foreign countries. Only the United States Department of State has the authority to grant, issue or verify U.S. passports. Those that may need Tourist passports will have to arrange for those passports through the main Post Office out in Jacksonville. The passport Office there is open from 10:00 a.m. through 1 p.m. Their phone number is 346-5061. A birth certificate with a raised seal or previous passport, 2 photos, and an application (DS-11) will be required as well as $67.00. (note: no DD Form 1056 required).

Some individuals have been identified by their sections to receive Government passports for the upcoming deployment. For those that need Government passports, the following instructions are provided:

Step 1. You must be identified by your section head as a Marine requiring a passport. In order to obtain a passport you must be 1) US citizen AND 2) have either an old passport or a birth certificate with a RAISED SEAL.

NOTE: If you have lost, can't find, or don't have a birth certificate with a raised seal, and don't have an old passport, you need to contact the clerk of records for the city or county you were born in and get a duplicate birth certificate WITH A RAISED SEAL. You can do it yourself by contacting the clerk of records OR there is a service online that will expedite that (for a fee). The web address is www.vitalcheck.com. If you are in this boat you need to order your birth certificate ASAP since it will take possible several weeks for that to arrive.

Step 2. You must go online and complete and print an application. Go to website https://pptform.state.gov . Go to bottom of the page. If you do not have a previous passport in your possession to submit, you must complete the DS-11. If you have a previous passport in your possession then most likely you can use the DS-82. Click on the appropriate form and complete the required information.
For Mailing address put the following: 

Bldg 233, F Street, 
Camp Lejeune, NC 28542 
For Permanent Address put your address. (This can be Home of Record if you wish) 
Leave "Travel Plans" blank

Once your info is entered push the "Create" button at the bottom of the questionnaire and it will create the appropriate form. Print off the appropriate form. (Note I only need the 2 pages of the actual application. I do not need the instructions printed off) Before you print it off, make sure it is correct or you will have to start all over again. Make sure that the printed form is legible. DO NOT SIGN THE FORM.

Step 3. Bring completed DS-11 , (or DS-82 if you can use that form) that you printed off, to 26 MEU SJA office along with Birth Certificate or previous passport. The SJA office will then type up a DD 1056 authorizing you to receive an official passport and give you the DD 1056 with 4 copies.

Step 4: Go to base photo. (Training and Support division 451-4262 Bldg 54, Behind Andy’s) Take a copy of the DD1056 and a civilian collared shirt. They will take (2) passport photos.

Step 5. Take the UNSIGNED DS-11 or DS-82, the original DD FORM 1056 (with 4 copies), your birth certificate (or previous passport), and two passport photos to the Military Passport Agent located in Building 233 on “F” street. (Next to the Camp Lejeune Bus station). Hours are 0800-1100 and 1300-1500. 451-9509. There the documents will be received and your application will be sent.
